COMPLETING YOUR NOTARY APPLICATION FORM Read all
instructions. If your form is not a completed , you
will be asked to resubmit the form.

1. You
can complete the Texas Notary Form online. To open and complete the form, you will need to
have a copy of Adobe Acrobat Reader on your computer. If you do not have a copy, go to the Adobe
website by clicking the icon below and follow the instructions for
installing the Reader on your computer.
2. Click the link at the
bottom of this page to open the form. Fill in all of the blanks and make sure that your form is complete before you
begin printing. All incomplete or unsigned forms will be returned to
you for resubmission. You may also wish to save a copy of your
completed form for your records.
3. After you have finished
typing in all of the required information, check your responses and
click the PRINT button at the top of the page to print the form. The form MUST be
printed on 8 1/2 x 11 white paper.
NOTE: Print an additional
copy for your records. The information you enter on the form will not be
saved if you try to save the form on your computer.
